Home Care professionals work as a team to deliver a complete range of services to homebound patients. Working with your physician, the team includes registered nurses, licensed practical nurses, home health aides, physical, occupational and speech therapists, and medical social workers. Home Care often provides an important link in recuperation from your hospitalization to your independence at home.
The Home Care team evaluates each patient and creates a plan of care. The plan of care may include services such as:
