According to national patient satisfaction surveys, Livingston HealthCare Home Health is ranked among the top 25 percent of home health programs nationwide. Surveys submitted in 2015-16 show high marks in patient care, communication and likelihood of recommendation to peers. The surveys are conducted by Home Health Consumer Assessment of Healthcare Providers and Systems (HHCAHPS), a standardized, publicly reported survey of patients' perspectives of home health care.
LHC Home Health team is made up of nurses, physical, occupational, and speech therapists; nurses, a social worker and home health aide. This team visits homebound patients who have been recently discharged from the hospital for doctor-prescribed treatments that may include: ongoing therapy, wound care, fall prevention, cardiopulmonary assessments, chronic disease education, and catheter management.
In 2015-16, LHC Home Health estimates their health team visited 6,123 patients in Park County from Wilsall to Cooke City, covering over 35,000 miles in rugged terrain.
LHC CEO Bren Lowe says the designation is evidence of the organization’s overall commitment to patient care. “The care we provide our patients doesn’t end when they are discharged. In vast, rural areas like Park County, ongoing communication and follow-up care can significantly impact the likelihood that our patients will continue to get and stay healthy.”
