Community Health Implementation Plan
Livingston HealthCare partnered with the Park County Health Department to carry out the Community Health Needs Assessment (CHNA) in the spring of 2022 to determine the most important health needs and opportunities for Park County, Montana. “Needs” were identified as the top issues or opportunities rated by respondents during the survey process or during focus groups.
The implementation planning committee—comprised of LHC's Leadership team—identified the most important health needs to be addressed by reviewing the CHNA, secondary data, community demographics, and input from representatives of the broad interest of the community, including those with public health expertise.
After reviewing all the data, the Community Health Implementation Plan (CHIP) was created. The CHIP prioritizes our communities health needs as determined through the assessment process and addresses the following healthcare issues:
- Behavioral Health
- Community Wellness: Housing and Income Inequality and Children and Families
- Access to Health Care
In addressing the issues, Livingston HealthCare seeks to improve access to healthcare services, enhance the health of the community and advance medical or health knowledge in our community. The CHIP serves as a roadmap for a healthy tomorrow.
